I believe that the price of the ETF is based solely on the bids and asks in the market. A sale occurs when a seller has an ask that is the same as a buyer's bid. There are brief moments when the the price of the ETF is slightly higher or slightly lower than the actual true value of the ETF's holdings, but a computer trader relatively quickly identifies the discrepancy and takes advantage of the difference. The computer trader's trades tend to bring the price of the ETF into alignment with the true value of its holdings.
