to my limited ins knowledge...no payments from medicare etc. I sent bills;pt paid bills; and then some sent to ins co. I did not do it except:

The only insurance I dealt with was from Delta Dental , a company started by the Massachusetts Dental Society for businesses to cover key employees. A dentist could sign up for it and their patients would be covered up to a point. The dentist would agree to allow Delta into the office to inspect records of Delta pts as opposed to other insurance patients as opposed to non covered patients...to make sure fees were consistent tween the 3 classes

I was not allowed to select the records; they did it at random when they came to my office

All others paid cash check or credit cards
