Quality only happens when there is constant pressure from a higher power. Quality is hard, takes time, costs money, and generally only positively impacts profits in the very long term view. As a result, most people won't do quality unless you constantly twist their arms. Also, bean counters love to cut quality as they see it on the books as an expense with 0 return.
