It is not different from clinical trials for cancer. A sick person comes in, they agree to participate in the clinical trial, and they are randomized into one of multiple treatment options. Which may [or may not] include placebo. If it is determined a treatment option is so efficacious that it is unethical not to offer a given treatment to the entire study, the study will be stopped and the offer will be extended. This is done all the time.
