I am no expert here, but one definite difference is that FTEL has a voice server product that allows phone calls over the Internet using regular phones, not a webphone device that connects to your PC. That would be part of the difference in quality, IMO. The other part would be how the digital/analog conversion is done via the voice server and how the packets are separated and reassembled.
