As I understand, Biden used to keep in pretty good physical shape. My guess is that his cognitive decline coupled with the rigors of a campaign (yeah, I know he is hardly campaigning but probably making a lot of calls asking / extorting money) are exacerbating his seemingly frail condition.

Many people have commented on how terrible a President looks after their term in office. It seems to age them. In reality, a President is more like a queen bee, a prisoner in her tribe than a leader. Everyone is constantly vying for your attention (or to get you to lay eggs). I suspect that despite Biden's total lack of visibility, his handlers are pushing him a lot with considerable preparation for even the shortest interview, BTW PDJT broke that mold.
