The method of use differs between here and europe at present.

In europe the value is encoded into a magnetic strip on the back of the card. The card is then inserted into the phone as you would a credit card. Australia is currently testing the debit card which is a chip based version and allows loading of money from ATM's and PC's. You get a piece od hardware at home and can load the card with cash from your bank on-line. The card can be used in vending machines and is therefore very convenient for small cash transactions. I believe this is the type of card that DCI plan to get to with the privilege card.

In the US the majority of cards require you to call a number and give a PIN number and your time starts then. The advantage of this over your calling card linked to you home phone is that there is no number on the card and normally no connection fee. You will notice that using your home phone calling card is expensive per minute. The cards have a pre-paid amount on them and can in some cases be reloaded by calling a number and using your credit card.

I do not know much more than this, most of my knowledge is picked up from doing my due dilligence so is pretty limited.
