Dear Harry:

I don't think I can explain it that well but I will try. Basically, insulin makes sugar in blood go to the rest of the body where it is consumed as energy, while amylin smooth the intake of sugar from digested food into blood. Both insulin and amylin get secreted after meal in normal people. Diabetic patients either can't make them (type I) or don't respond to insulin very well (type II). In theory, the diabetic conditions can be very well controlled by injecting the right amount of insulin.

But, it can not be done in real life. Too much insulin can cause blood sugar level to drop to such a point where brain doesn't have enough energy to consume and you will have an insulin shock(pass out). It can cause death. Most aptients inject insulin at a sub-optimal level to play it safe so no chance for insulin shock to happen.

However, this strategy will make the high peak of after meal blood sugar out of control. Amylin comes into play here, it slows down the flow of sugar form stomach to blood, just like to cut the peak and fill the valley. The result is a much more smooth and constant feed of sugar into the blood where sonstant lisulin level is maintained by injection. The end result is better overall blood sugar control. If you have to imagine, think amylin as a stretcher, it stretches a twisted wire straight, so it can fit into a small fixed-size tube.

And yes, I think amylin will be used to decrease the amount of insulin taken by patients once approved. Although that was where tha trouble was in last trials.

I hope I made sense.
