Note: 0.4 mg refers to twice the usual (prophylactic) dosage per kg of body weight. For a 120 pound woman (my guess), this would be two 12 mg tablets of IVM (scale for your weight, e.g. for 180 pounds, 180/120 = 3 12 mg tablets). Also you should continue this dosage for 5 days in a row even if you feel better after 1.

From page 2 of the actual protocol document:

lvermectin

2 0.4–0.6 mg/kg per dose (take with or after a meal) — one dose daily, take for 5 days or until recovered. Use upper dose if: 1) in regions with aggressive variants (e.g. Delta); 2) treatment started on or after day 5 of symptoms or in pulmonary phase; or 3) multiple comorbidities/risk factors.
