The US war doctrine is to minimize the US casualties at all costs, which necessitates excessive (Collin Powel called it "overwhelming") use of force. And that in return causes a disproportionate amount of collateral casualties, regardless of what the propaganda says. There is just no way it could be otherwise and anyone who thinks of it logically can see that.

Eventually some American generals came to understand that this over-emphasis on self preservation is one of the key reasons that the US wins every battle but loses the war. They just had a very hard time selling it to the Pentagon.
