Short on time but here goes the basic explanation, a true orthopod can elaborate:

1. intra=articular: in the joint space where the fluid would normally be found after being produced by the synovial membrane(look like white fronds or sea-anemone like projections). IE., you put eye drops in the eye not your nose even though they are close and juxtaposed. Injecting into the synovial membrane or hamstring isn't going to help.

2. chondromalacia: erosion on the undersurface of the patella(kneecap) which maybe a precursor to OA. If it doesn't track well with bending then it can erode and cause pain, etc. Sort of like not aligning or balancing your tires, 20,000 miles later they are bald on one edge. This may also develop from lack of synovial fluid so it would be one disease that would be benefited by an injection.

3. if you get bone on bone, the disease is too far advanced and I would think it's time for a total joint replacement; you've worn out the break pads so to speak, car still functions but noisy and risky.
