"These were the systems there supposed to protect the public"...

Poking at the logic applied in "making obvious mistakes" this week has been interesting...

"Reason" has an unfortunately not that short list of examples, which they have packed into 10 brief episodes of "Great Moments in Unintended Consequences"... again, showing that while not all systems appear equally capable of the same extremes in error... no one seems immune from the potential to make really bad decisions... the "tendency" appears universal...

Volume 2 is interesting in showing that, when you don't know what you're doing, even what appear to be very "small" decisions... can have not small consequences in extreme disproportion...

But, is it helpful... pointing it out... ?

Why are some systems more capable than others of making "really big" errors... and not correcting them ?
