Practically speaking, it means that the person is now a female. A person's choice determines their gender. In a way, they have become a freak of nature, biologically speaking, since they now possess characteristics of both genders. The Greeks referred to that as being a hermaphrodite, a man-woman. Since free will is higher in spirit than nature or biology, then the choice reigns supreme. It is all a question of values. Religious opponents always think they know what G-d thinks, values or desires. But in fact they are just imposing their own values and interpretations on G-d.

I see absolutely nothing wrong or immoral in a person changing genders. Actually, the transgender operation incorporates characteristics or features of the other sex so the person is neither fully man or fully woman. The person is now a man-woman. But the designation pays homage to the act of free will.
