Looking at the photo linked in ERIC's post, looks like most (all?) the mounting lugs on the fuselage are intact. My guess for a contributing factor is missing/compromised attachment hardware. As is usually the case there are several issues which combined together to liberate the plug. (and no the plane wasnt likely to crash due to this failure .... without other additive issues. re: Aloha flt 243 - testament to how robust modern planes actually are.)
