So you are saying the plane most likely was not built correctly? Missing attachment hardware sounds like dangerous and sloppy build work. As to crashing, I was thinking about what if this happened at 35 thousand feet and cruising speed. Could it have taken out a bunch more 'metal'? Who knows. I do remember that Hawaii flight, which was incredible when you actually saw pictures of the missing structure. It was great that nobody was actually sitting in the seat next to the damage. If they were it could have been an 'Auric Goldfinger' situation.
