Mike,
With a limit order it is confirmed whenever its filled. I usually watch the tape and when my limit price is hit I get the confirm. I rarely put a position on with a limit but many times I will enter a limit order once the market order is filled to close the position so I won't change my mind when the move comes thinking if I hold a little longer I will do better. Most of the time your first instinct is right and if the limit price is hit momentarily the order is entered. Of course the MM can see the order and come and get you and thats all right with me because I want to clear the position anyway.
