Good points.

The moderator will need to have the temperament -- and patience -- to suffer through a lot of sh*tposters and respond, not just exercise the ultimate sanction. Otherwise, it's potentially just censorship, not moderation.

Another excellent forum I belong to has a longstanding unofficial but well-understood policy of public warnings, and even then temporary (e.g. 7-day or 30-day) "timeouts" before permanent bans. By the time somebody reaches the end of the rope, everybody knows why.
