A critical question is whether the cancer is spreading or not. Imaging has been the standard technique but there is a new technique that is showing promise, delivering early results with very good predictive quality. Earlier results means the cancer can be treated before it spreads (much) and good statistics means treatments will improve with better understanding of what is and isn't working.

The company Personalis takes a sample of the tumor and a sample of the whole body and preforms whole genome sequencing on both. Those full data sets are compared and a subset of differences are chosen to watch. Then those are made into short DNA strands that will capture their complements from the blood samples. The filtered out DNA is then sequenced to get the answer, cancer spreading or not?

Roughly 6-12 months before the cancer was being detected using imaging, quite a lead on treatment. Very good sensitivity, down to 1 ppm of circulating cancer DNA. Quantitative results.

  • Early detection of lung cancer recurrence – 6 to 11 months ahead of standard imaging and significantly ahead of other assays
  • Identification of low and high recurrence risk patients which could lead to improved therapy decisions
  • Predictive of 5-year RFS and OS (Relapse Free Survival and Overall Survival)



This is a five year graph of Overall Survival. Green means they are saying "Clean" and look at the survival rate out to 5 years. Gray means cancer detected and lower survival rate. Check out the p value, 1.02e-12, it was not by chance.

Sequencing didn't help as much as expected with cancer, this is an improved technique that seems to give very useful information. It could indicate if treatment is working much earlier and more absolutely.
