Primarily because AD is such a tough nut to crack. The idea that this small molecule would have a dramatic impact on the disease strains credulity. The MOA remains rather nebulous and hard to really wrap my head around. The fact that so many choose to remain on the drug IMO reflects that the drug has no real side effects and the power of placebo.
