I worked in Nuqular and all events had multiple failures that led to the problem. Its never one thing. Instruments fail but there are backups and the human element to compensate for the failure. It always comes down to the human element. Having said that this airport looks very tricky due to traffic and lighting issues so anyone flying there had better be performing at the top of their ability. Looks like that wasn't the case
