Sorry, i think you're absolutely wrong here.

Its the doctor's legal, moral, and ethical responsibility to give safe treatment to a patient based on their systemic medical history, and the problem they present with at the time of the exam.

I believe most doctors will look at patients with hypertension and diabetes, not to mention other systemic problems, such as pulmonary and renal disease, for example, and will be very hesitant to prescribe a systemic drug. Regardless of what the labelled contraindications or PFE says about their product.
