For all you med sleuths out there. If you found this in the medical records from the Surgeon to the Home Health Nurses 2 days after the discharge of a patient with aortic dissection surgery....what would you expect to happen over the next week of that patients life? Oh yeah, you, the Surgeon already know the patient is anemic, is tachycardic, has oxygen saturation of 86% if off oxygen for 3 minutes, UTI, partially collapsed lung and a few other post surgery issues.

“Patient with unstable respiratory status: prescribed CHANGES to current plan of care due to need for oxygen management requiring skilled assessment, instruction and evaluation; assessment of efficacy or complications of prescribed CHANGES to the plan of care. Patient with new diagnosis of bilateral pleural effusion requiring prescribed treatment regimen CHANGES and need for plan of care for instruction and evaluation of efficacy/complications of CHANGES to treatment regimen.”
