I came to similar findings with Grok. Seems if you rewrite the question with just a little different approach, the Grok response can be the difference between life and death. Grok is always a valid source to get your education and direction.

I was always wondering why an internist that should have been related to this case did not appear to contribute any input in the medical records.

You are correct in that the patient was far more stable just before discharge than 2 days later at home. This reduction in stability prompted me to question why there was no readmitting or exam suggested perhaps at the hospital or an ER.

In the real case, no additional care was given to the patient and the family and the patient did not know of the declining health issues. When the nurses returned, they simply did the basic vitals check. They saw the deterioration but said nothing.

The patient was seen to be cyanotic just moments before death 3 days later. I suspect leakage at the repair site as the the patient bled excessively for several days after surgery.

Given the risks of known complications and clotting history I fail to see why the patient was not readmitted for tests/examination or suggested that the family visit and ER.
