Household income adds the income of every inhabitant of your house. 2-income, 3-income, or even 5-incomes or more can be included in household income.

At one point 2-years ago, we had 5-incomes in our household with my wife, myself, and my 3-kids who had jobs to support themselves in college.

You could have 3-people in a household who make $50k each, be middle income, and the household income will be considered the top 20% in the US.

Like Barbie said, math is hard.
