On the Yahoo NPTH thread some have questioned my attempt at understanding the economics of AutoPap particularly with regard to switching from secondary to primary screening. The question relates to the current use of AutoPap for secondary screening. I was assuming that only 10 percent of the slides were scanned by AutoPap for quality control purposes. A couple of the posters over there believe that all of the slides are scanned for quality control purposes and that 10% of these are selected by AutoPap for a second human screening.

Call this Approach A (my assumption) versus Approach B (the way I believe was described by two posters on Yahoo).

With Approach B the relative attractiveness of primary screening compared to secondary screening is certainly higher than would be indicated by my calculations. But it makes secondary screening look much worse to me. I can see the health benefit but it looks like all cost and no cost savings.

The overall effectiveness of primary screening I don't think changes if you assume Approach B because this assumption doesn't change the way primary screening is done.

Does anyone on this thread really know how AutoPap is used for secondary screening? In particular are all slides scanned or just a 10 percent sample?

I think it would be useful to have this clarified.
