Most high rise buildings in Manhattan are served by fiber at least partially.

T-1's are run via 4 twisted pair, a transmit pair and a receive pair. I have rarely seen Telco's run coax for any high speed applications either twisted pair or fiber depending on the speed involved is the norm.
