as far as the $1500 cap on part B outpatient reimbursement it is a small fraction of outpatient revenue vs. managed care, no fault, workers comp, medicare part A, etc.
medicare A(hospitalization insurance) is when someone has at least three day hospitalization for example a total knee replacement, stroke, coronary bypass, amputation etc
medicare B(health insurance) no three day hospitalization required for example you go to doctor with back pain and he sends you to physical therapy.
