Could someone summarize the JAMA article. I haven't been able to locate it. IMO, a lot of political pressure is being brought to bear on insurance carriers. For example, a candidate for governor of California is running a very effective television commercial in which a doctor complains that the State of California licensed him to practice medicine but bureaucrats in insurance companies won't let him practice effective medicine.
