Mike, when I attempted to exercise a call at Fidelity, they had trouble finding anyone there who knew how to do it. It turns out that you must personally call at the moment you wish to exercise (no setting the date and time in advance), the brokerage firm then must wire the options exchange, etc.

I was trying to save a few bucks in commissions but the commissions are actually higher because -- although there is no commission on the execution itself -- you pay undiscounted commission rates on the shares received.
