It would seem to me that the possibility of infection would be introduced by the presence of bacteria, dirt etc on the skin or clothing. If the outer clothing at the sternum was moved and the skin cleaned with a sterile swab as I'm sure all EMS personnel would carry, there would be only a limited chance for the introduction of infection. This assumes that the FAST 1 is sterile and effectively seals the puncture point.
Just a guess.
