I was in a similar situation, as I have put down on my certificates limit orders only, as I didn't recognize that it is applicable for market orders only to get back the fee for 5 trades.
That's why I didn't got credited anything on my account, after sending in the appropriate form.
After sending a e-mail message with market orders only within the period of 90 days, $40 was credited on my account.
