It works by a very simple biological mechanism. There is an identified chemical that makes the blood vessels relax and open up. This chemical is produced when one becomes sexually excited. Then there is a an identified antagonistic chemical that shuts the first one off. Most biological actions have an action and an opposing action. Remember anabolism and catabolism, or simply the muscles in ones body? So this drug interferes with the chemical that makes the blood vessels contract and return to normal size. Thus, it doesn't do anything for people who are not sexually excited. But it blocks the chemical that automatically works to tighten and close up the blood vessels.

It was a serendipitous discovery made when the drug was being used to treat something else, and it produces noticeable effects in 60-70% of the men (and perhaps women too) who have tried it.

Being in the medical field you probably have access to some better information than I have.
