I can't answer your first question, but crossover is when they switch cohorts between the drug and the placebo.

In a double-blind crossover study, every individual starts with either the drug or a placebo, but neither the subject nor the administering staff know which it is. Crossover occurs when placebo is switched to drug, or vice versa. All individuals are thus evaluated on *both* drug and placebo.
