Let me answer your questions this way:

1. The US FDA only requires the final step be done in a FDA inspected facility. Thus, if a manufacturing process involves 12 steps, the product from step 11 could be technically considered as raw material and be imported as such. In fact, most large pharmaceutical cos take advantage of the cheap labor cost of offshore countries and import as much intermediates/"raw materials" as possible to minimize costs.

The India company (or other houses) may very well supply US with products prior to final step and perhaps the final product to other countries depending upon local regulations.

2) In due time, PFE will maximize formulation advantage to create a version of VIAGRA that is fast acting (short onset time) and longer acting. Whatever side effects there may be could also be minimized by doing so. This, I think, could give PFE's VIAGRA an added marketing edge because advanced drug delivery systems (usually) could not be easily reproduced without proprietary technology. (PFE is currently collaborating with an Irish firm to accomplish this, I believe.)

PS: In fact, it's kinda ideal for a drug to have some side-effects that could be controlled. What happens is that the "ilicit" version of the drug would be more toxic (because of inferior quality) and all you need is a couple of scares to discourage folks from the ilicit sources. Lots of historical examples supporting this.
