Frank, in some areas you already have two ways into the home: ISP from the local phone company and from the local cable company.

And large companies stand to gain a lot by being able to fax over the internet for starters. Some faxes are already send that way. Note Xerox's document center commercials.
And many large companies would be interested in VoIP if it saves them money. And the large telcos will either act/react to the changes.
