Angela, I appreciate the work you have done to help get this information out to us here! I received my W-9 last week. Unfortunately, IT was filled out by NIAR people about NIAR... my understanding is that it should have been filled out by ME and mailed back to them. There were no instructions included. I have no idea why they filled it out with their information! I guess I will have to call them sometime about getting this straightened out... but since my wife just had a baby things are a little whacko at my house. Could you please check on this for me? THANKS Mitch
