One point not covered in your excellent post is that Medicare and Medicaid have forced the health care system to play the role of Robin Hood. Medicare and Medicaid have never paid anything close to the true cost of the health care freight--not the least of which is the cost of paper shuffling you mention. Therefore, the various factions in the health care industry have been forced to make up the difference by charging more for all services--to those who are able to pay and those who are not able to pay. In other words, you are paying out of at least two pockets for Medicare and Medicaid.
