Current, standard medical release form ("Disclosure Authorization"): "I authorize any doctor, hospital, practitioner, pharmacist, clinic, other medical facility, or provider of health care, and Medical Information Bureau to provide (insert name of insurance company here) any medical or non-medical information, data or records you may have regarding me, including, but not limited to, my medical history and treatment (including, but not limited to, records pertaining to psychiatric, drug or alcohol use history, and my HIV STATUS AND TEST RESULTS)." (I ADDED THE CAPITALIZED EMPHASIS.) So, buyer beware and carefully read what you sign (IMO, you can always "strike-out" any part you don't agree with, and initial it). Don't always look for a separate "HIV medical release" form; it just may not be there. |
