The nuclear devices Pakistan and India possess are of Hiroshima size where as the ones the US and Russia possess are 1,000 times that size.
A nuclear exchange between those two countries would kill a couple of million people but the fallout to neighboring countires would be minimal. Unless of course the bankrupt Russians have been trying to raise hard cash by selling ICBM's.
