It is interesting to see the article in Urology.
We have used brachytherapy in our institution since 1977. We have used gold seeds placed freehand. Our results were very good and many patients had negative biopsies and undetectable PSA values. I have noticed that some patients are now recurring with rising PSA as far as 15 years post implantation. This suggests that an initial cancer may have been treated successfully and now the remaining prostate tissue has degenerated. The bottom line is that seeds may not be a good treatment for those patients who have a long term life expectancy.They do have a role for local control perhaps.
