Note that it is commonly accepted practice in most failed software projects to "blame the innocents", i.e., the programmers.

Management, including project management, and especially upper management, is, was, and always will be mainly responsible for overlooking this "missing requirement", which they have been repeatedly warned about from lower levels for years. This is a well-documented fact. Over the years, management's response has invariably been something in the order of, "don't bother me with anything that costs money this quarter, especially something so labor intensive, yet with no apparent, immediate payback".

I guess they were worried about their stock options...
