Blowout BUMMER!!

I'm somewhat new to oil stocks. I guess it doesn't take a rocket scientist to understand that this is bad news for them, but I have a couple of general questions about blowouts. Are they caused by human error or just bad luck? What makes wells more or less likely to have a blowout? I see that they are at least insured. What does the insurance do for them? How many other wells are they involved in right now? Where did this one rank size wise among the wells they are involved in?
