Actually, in general they do hold back from that kind of thing. For example, think of the IRA vs the British Royals and PM. The reason is, I suspect, simple - the people actually performing the terrorist acts are fanatics willing to die, but the people in charge are more political in nature and are not willing to die. And, they know that if they start assassinations their life expectancy isn't too long. Remember the Soviets in Beruit: they had one (or more? - I've forgotten) of their diplomats explicitly kidnapped and killed and lo and behold several months later several suspected terrorist leaders showed up dead with their testicles removed. No more Soviets ended up dead.

I suspect the prohibition against assassination is kind of like the old MAD doctrine - we won't if you won't.
