re: commissions on spreads

is there only one commission on a spread order? i assume on a call or put spread there is only one commission. what about a butterfly spread (a put spread and a call spread)? is this placed as one order or two separate spread orders? i assume two, and that there are two commissions. am i correct?

plus, if you are assigned the short option in a spread, how is it covered? by the long option? how many commissions does this require?
