Printing out a header

Does anyone know how to print out a header so that it appears
once and not repeatedly for a custom scan?

for example, i want something like this:

SYMBOL EPS QTR 1 EPS QTR 2 EPS QTR 3 EPS QTR 4
===============================================================
XYZ 3.30 4.50 0.45 0.34
ZYX 3.40 3.56 4.67 0.33
