Yes, parenthetical expressions must be set off by commas. It is true, though, that we don't tend to pause when we say them. It is the flow of thought that is pausing to explain or qualify.

Mike, we must be patient when we correct others, and not give occasion to make the messenger's attitude become a focus of debate.
That keeps things cleaner: Scalpels, not chainsaws are used in surgery.

I know, I know! Tree surgeons use them sometimes.

By the way, "To really be or not to really be . . ." Hmmmm, that's a good question.
