In a previous post, your opening assumption was that many people die instantly from heart attacks. I am arguing that in fact it is a low percentage (16.6) that die instantly.

I would not say that one out of every six first heart attacks resulting in a fatality is a low percentage. I wouldn't want to bet my life on those sort of odds.

The second point was that while the same "low percentage" of people develop heart disease up to 20 years before retirement , that is a huge market for these scanners. I am guessing there are at least 80 million adults 45 to 64 in this country so the number of people who develop heart DISEASE would be over 13 million people.

This is exactly why the HMOs won't want to pay for scanning. Most of these people suffering from heart disease won't even know it until they have their first heart attack, which they may never have. At what age are most first heart attacks? I'm assuming that it is 65+, when these people are now on government health insurance.

Of all the people who have heart attacks, what percentage have them under 65 and live? Every other heart attack represents someone the HMO won't have to pay for (either dead or 65+). Now figure that the HMOs will be paying for preventative care for everyone who has a test and tests positive, that is everyone who would have had a heart attack and many who never will, and you see that they'd save money by discouraging testing. And people who wouldn't have to die will.
