Just thought that I would throw in my two cents for what it's worth. Add-on business is really not that lucrative. The reason being, believe it or not, is the fact that these add-on levels are not written by the engine creators but rather amateurs who have taken the time to create them. They do it as a challenge and because it's fun. In fact, the Duke Nukem 3D CD has a Build directory that gives you the tools to do it. Also, I have bought an add-on for both Duke and Quake and I can assure you that one can find as good or even better right off the web. In fact, most of the add-on levels show the credits of the creators. I have enjoyed the web maps more than the add-ons. In the long run not much revenue from something that can be taken right off the WWW <IMHO>.
