The current huge problems in Australia after an explosion in a Gas facility (not caused by Y2k) clearly is an example of the impact of 'single point of failures' architected systems. For more than 1 million people there is no gas, and no alternative source of gas.

Same goes for the problems in NZ after a high voltage cable broke. Only with lots of outside help, the problems could be mastered after several weeks I think. Look at the master index, I have compiled a number of articles:
