What about a SCSI primary and an IDE KOT?

I like the idea. I think it will work but that does not mean it will. One needs to verify that the mobo bios has an option to select between IDE and SCSI. Most new mobo's have this option.

With one SCSI harddrive and one IDE harddrive you would make the boot selection in the mobo bios. I would still recommend a SCSI host adapter with boot target ID selection as you may want to add another harddrive in the future.

If this is a feasable solution I could also utilize the SCSI adapter for cd_rom now and cd-rw or DVD later.

The biggest advantage of SCSI IMO is the ability to expand your system. In addition to harddrives and CD xxx drives you can get tape, LS120, ZIP, scanners... SCSI is recommended for CDR and CD RW (IDE has made a lot of performance advances recently).

What is your time frame for your system? My biggest concern is KOT in the NT OS. I have never tried it. NT looks at hardware a little different than Win95/98. I have been looking for an excuse to load NT on my home machine.
